What is a Continuity of Care Document (CCD)?

• The Continuity of Care Document (CCD) is the result of a collaborative effort between the Health Level Seven (HL7) and ASTM organizations to “harmonize” the data format between ASTM’s Continuity of Care Record (CCR) and HL7’s Clinical Document Architecture (CDA) specifications. 

What is a Consolidated Clinical Document Architecture document (C-CDA)?

• The Clinical Document Architecture (CDA) is a popular, flexible markup standard developed by Health Level 7 International (HL7) that defines the structure of certain medical records, such as discharge summaries,and progress notes, as a way to better exchange this information between providers and patients.

What does this mean to you?

• The C-CDA will enable referring physicians, urgent care, and emergency departments to send the patient summary containing a core data set of the most relevant administrative, demographic, and clinical information facts about a patient's healthcare, covering one or more healthcare encounters.  It provides a means for one healthcare practitioner, system, or setting to aggregate all of the pertinent data about a patient and forward it to another practitioner to support the continuity of patient care. Its primary use case is to provide a snapshot in time containing the pertinent clinical, demographic, and administrative data for a specific patient.

More and more primary care physicians are embracing EMR technology that has the capability of producing a C-CDA, however, because most referral physicians do not currently have the capability of opening, printing or storing of the C-CDA, they are not sending it.  Once you have installed DocuMed’s CCD / C-CDA reader, you may notify your referring practices that you would prefer receiving patient information via electronic C-CDA.
